controversial because of their obscure mechanisms. A randomized crossover study, using iron lung and positive pressure nasal ventilation (BiPAP) each for 40 mm, was performed in 11 stable patients with severe COPD. Throughout the study, we monitored surface EMGdi, EMGst, ECG, Sa05, ETCO2, and the movements of RC and tB. Afterwards the data were replayed to calculate VT, RR, PR, VT/TI, iEMG, and phaseangle. No statistically significant improvement was found in view of the above parameters. However, the percentage of iEMGst change after 40-mm BIPAP ventilation, compared with the baseline, was much more significant in patients with FEY below 0.55 L than those with FEV above 0.55 L (n 4:7, @iEMGst —¿ 62.93percent ±23.27 percent vs 32.45 percent ±42.79

A comparison of leak compensation in acute care ventilators during noninvasive and invasive ventilation: a lung model study.
BACKGROUND Although leak compensation has been widely introduced to acute care ventilators to improve patient-ventilator synchronization in the presence of system leaks, there are no data on these ventilators' ability to prevent triggering and cycling asynchrony. The goal of this study was to evaluate the ability of leak compensation in acute care ventilators during invasive and noninvasive ven...
متن کاملEarly Effectiveness of Noninvasive Positive Pressure Ventilation on Right Ventricular Function in Chronic Obstructive Pulmonary Disease Subjects with Acute Hypercapnic Respiratory Failure
Introduction: Noninvasive positive pressure ventilation (NIPPV) has become an integral tool in the management of acute hypercapnic respiratory failure (AHRF) in chronic obstructive pulmonary disease (COPD). This study was performed to evaluate the early effects of NIPPV on pulmonary artery pressure (PAP), serum N-terminal pro BNP (NT-proBNP), and ventilatory parameters in the COPD patients with...
متن کاملIncreased ventilation with NiIPPV does not necessarily improve exercise capacity in COPD.
Evidence that noninvasive intermittent positive-pressure ventilation (NiIPPV) improves exercise capacity in chronic obstructive pulmonary disease (COPD) is limited. The effectiveness of different ventilators in this setting has not been studied. Three bilevel pressure support ventilators (Bipap S/T 30, Nippy2 and Vpap II ST), applied via a mouthpiece, were compared during submaximal treadmill e...
متن کاملClinical relevance of maximal inspiratory pressure: determination in COPD exacerbation
Muscle dysfunction represents a pathophysiological feature of chronic obstructive pulmonary disease (COPD). Muscle impairment contributes to decreased effort capacity in these patients at least in the same proportion as pulmonary function limitation. Maximal inspiratory pressure (MIP) is a reliable, noninvasive parameter for assessing the respiratory muscle capacity. The aim of the present stud...

WINNING ABSTRACT: From a total of 1211 adult & paediatric patients receiving home ventilation (HV) supervised by Royal Brompton Hospital between 1/1/06 and 30/6/06 the respiratory support team received an average of 528 daytime calls/month and 14/month out of hours calls to a telephone helpline. Diagnoses included: neuromuscular disease, chest wall disease, COPD, obesity hypoventilation and non...
متن کاملEffects of gas leak on triggering function, humidification, and inspiratory oxygen fraction during noninvasive positive airway pressure ventilation.
OBJECTIVES During noninvasive positive pressure ventilation (NPPV), the gas leak that commonly occurs around the mask can render NPPV ineffective. We evaluated the effects of gas leak on inspiratory trigger function during NPPV with bilevel pressure and ICU ventilators. In addition, we evaluated the effects of gas leak on fraction of inspired oxygen (Fio(2)) and humidification. METHODS Air le...
